Population Served - Individuals with prolonged mental illness, ages 18 and over

Group Home
* 6 bed community based residential facility
* individual, group and family counseling
* daily educational groups focusing on rehabilitation including medication, anger management, anxiety management, dual recovery, identification of emotions, social skills, shopping skills, and health and fitness
* supervised, structured activities of daily living
* discharge planning including referrals for suitable housing, financial support, aftercare counseling and continued mental health treatment

Residential Support
* 10 clients living independently in Waterbury
* individual and family counseling
* case management services
* individualized support and services
