Population Served - Men on Community Release status from the Department of Correction with mental health issues, ages 18 and older

Program
* 15 bed residential program
* 8 - 14 month length of stay
* individual, group and family counseling
* daily educational groups including addiction, depression, schizophrenia, anxiety, relapse prevention, healthy relationships, communication and anger management
* supervised, structured activities of daily living
* vocational counseling and job search assistance
* Alcoholics Anonymous, Narcotics Anonymous and Dual Diagnosis meetings
* medication assessment by a nurse practitioner
* discharge planning including referrals for suitable housing, financial support, aftercare counseling and continued mental health treatment

Admission/Referral Process
* Department of Correction counseling or mental health staff
* Department of Correction Parole and Community Services Unit
* the client may contact either of the above entities
