Population Served - Individuals with prolonged mental illness, ages 18 and over

Group Home
* 6 bed community based residential facility
* individual, group and family counseling
* daily educational groups focusing on rehabilitation including medication, symptom management, relaxation skills, community resources, recovery, budgeting and financial management, social skills, and relapse prevention
* supervised, structured activities of daily living
* discharge planning including referrals for suitable housing, financial support, aftercare counseling and continued mental health treatment

Residential Support
* 20 clients living independently in Connecticut's Northwest corner
* individual, group and family counseling
* case management services
* individualized support and services
